Portland, Oregon, United States Providence Full time

As a doctor in Providence ElderPlace in Portland, you'll be part of a dedicated team offering older adults a coordinated program of complete care and social services. Successful candidates should have recent experience working with elderly patients and be board-certified/board-eligible in Family Medicine or Internal Medicine, or have completed a Geriatric Medicine fellowship. In this role, you'll manage a small group of patients at your main practice location and support other ElderPlace locations in Portland when needed.   • Part-time position: 32 hours per week, 0.8 FTE, working 4 days a week, Monday to Friday, daytime hours.
  • On-call rotation: 1-2 weeknights per month, 1-2 weekends per quarter.
  • Responsibilities include outpatient primary care, home visits, and community-based visits.
  • Collaborate with a team including nurse practitioners, nurse care managers, social workers, pharmacists, and behaviorists.
  • Required: 2 years of recent experience with the geriatric population; preference for Geriatric or Palliative Medicine fellowship.
