Portland OR, United States Provider Solutions & Development Full time

As a physician with Providence ElderPlace in Portland, youll join an established interdisciplinary team providing older adults with a single, coordinated program of comprehensive care and social services. Successful candidates will have 2 years of recent clinical experience working with a geriatric population and be board-certified/board-eligible in Family Medicine or Internal Medicine, or completion of Geriatric Medicine fellowship. Part-time, 32-hours per week, 0.8 FTE, 4 days per week, Monday - Friday, daytime hours
~ Call schedule 1-2 weeknights per month, 1-2 weekends per quarter
~ Outpatients primary care setting, home visits and visits in community-based settings
~ Geriatric or Palliative Medicine fellowship preferred
