Outreach & Enrollment Representative

Location: Onsite, San Jose, CA

Pay Rate: $30-35 per hour


Job Responsibilities

· Cultivate and sustain relationships with current members and key community stakeholders while facilitating referrals to surpass monthly enrollment objectives

· Engage in community outreach events and meetings to stimulate referrals.

· Formulate and implement enrollment strategies tailored for new target demographics.

· Collaborate and coordinate with operational personnel to ensure an efficient and welcoming enrollment procedure.

· Oversee and participate in all facets of the enrollment assessment process as required to ensure punctual enrollment.

· Timely completion of all data collection and mandatory documentation during the enrollment process, adhering to specified timeframes.

· Maintain an updated database containing information on all enrollment and outreach initiatives.


Position Qualifications/Requirements:

· 1+ years' experience within Healthcare or Senior Living

· Ability to work with limited supervision

· Excellent time management skills and ability to coordinate with all levels of staff and handle multiple tasks from various office departments

· Strong proof reading, formatting, and editing skills and grammar

· Valid drivers license

· Bachelor's Degree preferred, not required
