The Medical Director of Preventive Medicine provides clinical administrative leadership regarding preventive health. The Medical Director of Preventive Medicine is responsible for creating and implementing strategies to address preventive health quality of care provided to participants in Primary Care at Howard Brown Health. The Medical Director of Preventive Medicine will promote the current policy and procedures for Howard Brown Health Center in alignment with the clinical management team. The Medical Director of Preventive Medicine is a partner to other departments and works in collaboration with operational initiatives.


PRINCIPAL DUTIES AND RESPONSIBILITIES:


As clinical administrative leader:

  1. Apply knowledge of general preventive medicine and public health issues to promote health care to groups or individuals, and aid in the prevention or reduction of risk of disease, injury, disability, or death. May practice population-based medicine or diagnose and treat patients in the context of clinical health promotion and disease prevention.
  2. Design, implement, or evaluate health service delivery systems to improve the health of targeted populations.
  3. Coordinate integration of physicians, nurses, statisticians, or other professional staff members.
  4. Identify groups at risk for specific preventable diseases or injuries.
  5. Design or use surveillance tools, such as screening, lab reports, and vital records, to identify health risks.
  6. Develop health assessment methods or programs.
  7. Train medical staff regarding preventive medicine issues.
  8. Evaluate the effectiveness of risk reduction measures or other interventions.
  9. Integrate resources offered by internal and external agencies, or other organizations to improve community health.
  10. Work with QI to prepare preventive health reports including problem descriptions, analyses, alternative solutions, and recommendations.
  11. Develop or implement interventions to address behavioral causes of diseases.
  12. Design and/or manage health prevention programs
  13. Work with communications to deliver health and wellness information internally to organization and to the public.
  14. Document or review comprehensive patients' histories with an emphasis on occupation or environmental risks.
  15. Build strategies for patient health information gathering and chart updates
  16. Teaches and develops educational CME programming as needed
  17. Collaborates with the Integrated Health Care team in program planning, monitoring and evaluation of preventive health services including collaboration with Patient Centered Medical Home and Meaningful Use at the clinical site and clinical quality initiatives.
  18. Provides direct care for patients at the sites according to percentage of allocated effort:1.0 FTE will include 24 hours per week for Medical Director of Preventive Medicine, 16 hours per week of face to face clinical time.
  19. Evening, weekend hours are required.
  20. Medical Director of Preventive Medicines are required to enter the telephonic call schedule rotation.
  21. Attends and participates in the Medical Leadership weekly meeting, clinical quality meetings as required and other leadership meetings as required.


As Clinician:

  1. Facilitates entry of the patient into the health care system, either at the HBHC main clinic site, homes or community shelters or the hospitals where the Medical Director has admitting privileges by assessing the primary health care needs, including eliciting comprehensive health histories, performing physical examinations, ordering and/or performing pertinent diagnostic tests, analyzing collected data to determine client health status and formulating a problem list (diagnosis.)
  2. Provides coordinated, continuous and culturally competent primary health care to clients that span the health continuum through mutual goal setting and planning delivery of care.
  3. Prescribes a variety of treatment modalities to meet health and illness needs.
  4. Meets provider productivity standard including but not limited to number of visits per week

5.Collaborates with advanced practice nurses and community health nurses to assure that health teaching and counseling is provided.

  1. Will follow role assignments that are in alignment with the Patient Centered Medical Home Standards, and participate in all Patient Centered Medical Home efforts.
  2. Meets productivity standards including but not limited to number of visits per week
  3. Manages and evaluates the plan of care, process and outcome and then modifies the plan as needed.
  4. Adheres to collaborative practice principles of respect and joint decision making when: ◦Providing clinical supervision of the medical care delivered by the advanced practice nurses.
  5. Consulting with health team members about managing the client treatment plan.
  6. Following clinical practice standards (protocols) related to each health care providers role and responsibilities.
  7. Assures access and continuity of care by providing call for after-hours coverage.Assures continuity by obtaining hospital privileges at designated hospitals and by making home visits when appropriate.
  8. Seeks opportunities for community and professional outreach to maintain and increase patient referral base. Outreach activities can include, but are not limited to:
  9. Providing training and presentations on health care issues to multi-disciplinary professionals and community agencies.
  10. Attend community health fairs.
  11. Coordinates services in conjunction with other team members to meet patient’s primary health care needs and to ensure the continuity of care.
  12. Participates in quality improvement activities through protocol development and collaborative peer review by: ◦Developing medical and joint practice protocols and identifying appropriate evaluative criteria for utilization review activities.
  13. Evaluating care based on clinical standards through the quality improvement process.Implements recommended changes and/or improvements to guarantee quality care based on the evaluative process.
  14. Assumes the responsibility for professional development and contributes to the professional growth of others by:
  15. Developing in-depth knowledge of daily program operations, policies and procedures.
  16. Acting as a preceptor in the orientation of new health care personnel, interns, residents and volunteers.
  17. Serving as a consultant in specialized populations to other health care providers and community agencies.
  18. Incorporates and disseminates research findings as a basis for practice and to improve health care outcomes.
  19. Identifies clinical questions to be researched.
  20. Assumes the responsibility for maintaining competence in clinical practice by:
  21. Engaging in mentor relationships with experienced mid-level providers and physicians to gain clinical expertise.
  22. Developing and maintaining a patient panel as clinical competence is achieved to enhance continuity of care and meet Organization productivity standards.
  23. Participating in appropriate educational activities in order to maintain Board certification and continue professional growth.
  24. Provides cost effective Organization productivity requirements by practicing collaboratively with the advanced practice nurses and the community health nurses to prevent the duplication of services.
  25. Practices within the legal and ethical guidelines established by the Medical Practice Act, the Physicians Code of Ethics and the Specialty Standards of Practice.



SUPERVISORY RESPONSIBILITIES:

The Medical Director of Preventive Medicine supervises medical providers and other staff as assigned for their clinical location.


QUALIFICIATIONS, SKILLS AND ABILITIES:

Education and/or Experience in one of the following:

  • Graduation from a college or university accredited for Advance Practice Nursing.
  • Graduation from a Physician Assistant Program
  • Graduation from a college or university accredited by the American Medical Association with a Doctor of Medicine degree or accredited by the American Osteopathic Association with a doctor of Osteopathy degree. If medical education occurred at a foreign medical school, the applicant must possess a current Educational Council Foreign Medical Graduate (ECFMG) certificate. Successful completion of an approved program of residency training in family practice.
  • Board certification in Family Practice or Internal Medicine preferred.
  • Experience in establishing and maintaining effective working relationships with other employees, patients and the general public.
  • Experience with diverse patient populations and experience in community health settings


KNOWLEDGE:

Knowledge of the principles and practice of preventative medicine, state and federal laws pertaining to medicine and community health centers, and the structure and function of community health centers.


CERTIFICATES, LICENSES AND REGISTRATIONS:

Active Illinois Physician licensure, plus current DEA registration, Board certification in a specialty area (must be completed within two years of employment), documentation of a Medical Degree from an accredited school of Medicine, documentation of residency completion in a specialty area from an accredited residency program and Physician National Data Bank verification of legal claims history.
