PHP Clinician

Adolescent Partial Hospitalization Program - Lowell, MA

Bournewood Health Systems is seeking a Clinician for an Adolescent Partial Hospital Program. The incumbent’s prime responsibilities will be:

  • To provide direct clinical service to assigned Bournewood Health Systems Partial Hospital patients.
  • Inclusive of individual and family treatment interventions.
  • Case management, disposition planning, care consultation and utilization review.
  • Initial assessments for each patient and treatment planning.
  • Meets with other disciplines regarding patients' treatment plan.
  • Consults on an as needed basis with other team members around issues of his/her expertise, including case management and disposition planning.
  • Leads psychotherapy and/or psycho-educational groups for Partial Hospital patients.

Qualifications:

Candidates must have strong organizational and interpersonal skills, and the ability to multitask. Partial Hospital or Inpatient experience with adolescents is strongly preferred. Social worker requirements include the completion of a master's degree in social work from an accredited school of Social Work or master's in counseling and License eligible.

Job Description

Essential Functions:

  • Leads daily group therapy sessions as indicated by Supervisor.
  • Assumes role of social worker for assigned patients.
  • Meets with patients and guardians to complete the written initial assessment within 24 hours of patient admission, and completes treatment plan within 48 hours.
  • Provides and maintains contact/consultation, (as patient’s guardian permits) with collateral supports including but not limited to patient’s outpatient therapist, prescriber, PCP, school, DCF/DMH case worker.
  • Involves and updates families on treatment planning, treatment progress, aftercare planning and educates family on available resources.
  • Coordinates/organizes meeting of outpatient providers or school personnel as necessary.
  • Communicates findings and progress of group, individual, and family therapy and discharge recommendations/planning to treatment team.
  • Consults on an as needed basis with other team members around issues of his/her expertise.
  • Maintains daily progress notes in charts and completes group notes for all groups performed.
  • Ensures appropriate case management and aftercare appointments with providers and informs patient and family of all appointments.
  • Conducts utilization review on cases assigned by the Supervisor.
  • Documents all clinical and collateral contacts and case management activities in a timely and professional manner.
  • Complies with all statues and regulations relevant to the maintenance of clinical standards in the Commonwealth, e.g. mandated reporting, duty to warn, confidentiality, human rights.
  • Complies with the policies and procedures of Bournewood Health Systems.
  • Adheres to the Social Work Code of Ethics.
  • Performs other related duties as requested by the Supervisor.

Essential Competencies:

  • Knowledge of major mental illness, i.e. schizophrenia and the impact of family.
  • Knowledge of developmental stages and clinical implications regarding abandonment.
  • Knowledge and understanding of biological and sociocultural changes influencing adolescents, adults, and geriatric populations.
  • Knowledge of substance use disorders and associated treatments.
  • Knowledge of family systems dynamics.
  • Knowledge of group therapy and group dynamics.
  • Ability to conduct individual assessments and provide effective clinical documentation.
  • Is competent in delivering patient care that is trauma informed and patient centered. As indicated by:
  • Provides care that is kind, collaborative and compassionate.
  • Able to defuse power struggles and doesn’t need “to win”.
  • Appropriate assessment of clients for a history of trauma
  • Recognize/assess a range of trauma-related behaviors
  • Implement trauma-sensitive interventions with people served

Physical Criteria:

  • Must be able to climb stairs and walk between buildings.
  • Must be able to use a telephone and computer.
  • Must be to sit for up to 8 hours.
  • Must be able to provide crisis intervention techniques when necessary.
