Preferred Qualifications:

Completion of a Pharmacy Practice (PGY1) Residency or equivalent experience

Completion of an Oncology (PGY2) Pharmacy Residency or equivalent experience

Board Certification in Oncology Pharmacy



Day-to-Day:

The Neuro-Oncology Clinical Pharmacist Practitioner (CPP) functions as an advanced practice practitioner (APP) in collaboration with a multidisciplinary team to provide comprehensive pharmacist care services, disease management, and specialty medication optimization for adult patients seen in the CNS/Neuro-oncology clinic. This is done through innovative clinical services, continuity of care between acute care and ambulatory care settings, quality improvement and outcomes initiatives. The Carolina Board of Pharmacy designates the title of CPP to licensed pharmacists approved to provide drug therapy management, including controlled substances, under the direction of, or under the supervision of a licensed physician.


The neuro-oncology clinical pharmacist practitioner forms part of the Clinical Oncology Group within the Department of Pharmacy at Hospitals. The Department of Pharmacy is nationally recognized for excellence in pharmacy practice and has received numerous ASHP Best Practices Awards in Health-System Pharmacy. Team members are encouraged and empowered to pursue best practices in their specialties and contribute to innovative initiatives that advance practice. The embedded residency programs located at various hospitals– including a PGY2 Oncology Pharmacy Residency – and partnership with the UNC Eshelman School of Pharmacy provides ample opportunity for preceptor and research development.


Responsibilities:

Lead the pharmacy services in the neuro-oncology clinic as a CPP and collaborate with physicians and nurses via a hybrid work model. The CPP has the flexibility to operationalize the clinic and optimize the workflow to meet the needs of the neuro-oncology population.

Clinical Responsibilities

Function as a limited oncology prescribing practitioner (LOPP) to collaboratively manage chemotherapy orders. Provide comprehensive medication management for patients receiving chemotherapy for primary CNS malignancies

Manage oral and IV chemotherapy toxicities through hospital credentialing as an APP

Educate patients and their caregivers about their chemotherapy plan

Document patient encounters in the electronic medical record and with documentation of items necessary for facility fee billing

Coordinate medication-related aspects of transitional care

Serve as a liaison between the Department of Pharmacy, CNS/Neuro-oncology clinic, and Department of Neurology to assure all activities are coordinated and consistent

Support medication management goals set forth by the institution or Department of Pharmacy

Collaborate with the acute care pharmacists and the chemotherapy infusion pharmacy team on policies, procedures, and guidelines associated with this patient population

Serve as clinical liaison among clinical neuro-oncology team, UNC Medication Assistance Program, and specialty pharmacies (including UNC Shared Services Center Pharmacy) to help address medication access barriers and ensure timely processing and delivery of specialty medications
