About the Position: We are seeking a dedicated and compassionate Caseworker to join our team. The ideal candidate will have a Bachelor’s degree and at least one year of experience working in a hospital setting. This role is crucial in providing support and advocacy to patients and their families, ensuring they receive the necessary resources and assistance to navigate the healthcare system effectively. This position is an in-hospital only role. No home visits required.


Key Responsibilities:

  • Assess patient needs through interviews, consultations, and evaluations to develop appropriate care plans.
  • Provide emotional support, counseling, and guidance to patients and their families.
  • Coordinate with medical staff, social services, and community resources to ensure comprehensive care.
  • Assist patients in understanding their diagnosis, treatment options, and available resources.
  • Advocate for patient rights and facilitate communication between patients, families, and healthcare providers.
  • Maintain accurate and up-to-date case records, documenting patient interactions, care plans, and progress.
  • Conduct follow-up assessments to monitor patient progress and adjust care plans as needed.
  • Educate patients and families about healthcare policies, procedures, and available services.
  • Participate in interdisciplinary team meetings to discuss patient care and develop collaborative treatment plans.
  • Provide crisis intervention and support during emergency situations.
  • Stay informed about changes in healthcare regulations, policies, and community resources.


Qualifications:

  • Bachelor’s degree in Social Work, Psychology, Nursing, or a related field.
  • Minimum of one year of experience in a hospital setting.
  • Strong understanding of healthcare systems, medical terminology, and patient care processes.
  • Excellent communication, interpersonal, and organizational skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Compassionate and empathetic approach to patient care.
  • Proficiency in using electronic medical records and case management software.
  • Knowledge of community resources and social services available to patients.


Preferred Qualifications:

  • Bachelors Degree
  • Experience with crisis intervention and counseling.
  • 1 year Case Management experience in a hospital setting


Working Conditions:

  • The position may require extended periods of standing and walking.
  • Ability to handle high-stress situations and work in a fast-paced environment.
  • May require occasional evening or weekend hours depending on patient needs.
