Job Information


The clinical pharmacist is responsible for working directly with patients, prescribers, other clinicians, payers, and pharmaceutical manufacturers to optimize medication therapy and patient health outcomes. The clinical pharmacist is also responsible for fulfilling the responsibilities of the staff pharmacist. The clinical pharmacist may act as the temporary Pharmacist in Charge (PIC) when the PIC, Senior Clinical Officer, and/or Director, Operations are absent. The Clinical Pharmacist will potentially oversee the staff, clinical programs, policies, and procedures for the ambulatory infusion center (AIC) and home infusion therapy (HIT). The Clinical Pharmacist will also offer clinical oversight, guidance, and support to the sales and marketing departments.


Job Summary


The clinical pharmacist oversees the provision of the OptiMed patient management programs (PMP) and aims to improve patient-focused clinical outcomes using evidence-based resources, enhance patient understanding of their disease process and associated therapy, increase patient adherence to their medication regimen and treatment plan, and prevent medication misuse and detect and prevent adverse drug events. The clinical pharmacist performs comprehensive counseling with each patient assessing the patient for potential adherence issues, and educating the patient on appropriate medication use, storage/handling requirements, potential side effects, and any other requirements or pertinent information related to the patient’s therapy. The clinical pharmacist is responsible for developing care plans, setting treatment goals, and the ongoing patient monitoring for treatment success and/or side effects as outlined in the applicable clinical protocols. The clinical pharmacist also serves as a drug information resource for patients and providers participating in clinical or specialty programs. The clinical pharmacist will work with the Senior Clinical Officer and the Clinical Oversight Committee to develop, monitor, and edit treatment protocols and new clinical programs.


Essential Duties and Responsibilities


The duties and responsibilities of the clinical pharmacist include but are not limited to:

  • Developing clinical pharmacy programs according to policies, regulations, and accreditation.
  • Reviewing prescriptions to ensure accuracy and appropriateness of medication therapy.
  • Developing effective medication plans and optimizing drug therapies that minimize the risk of adverse effects for patients.
  • Educating and advising patients, caregivers, and prescribers regarding correct administration of prescribed medications, drug interactions, side effects, dosage and proper medication storage.
  • Analyzing trends to monitor patient adherence and to prevent excessive use or harmful interactions.
  • Advising patients on the selection of over-the-counter products, supplements, and other health care supplies.
  • Administering immunizations or other medications requiring administration by healthcare professionals.
  • Referring patients to other health professionals and agencies when appropriate.
  • Maintaining accurate documentation of medication plans and patient progress within patient profiles.
  • Maintaining an accurate inventory including proper storage and handling.
  • Continuing his/her education regarding new drugs, medical breakthroughs, innovations, and ideas on new practice models.
  • Teaching pharmacy students serving as interns in preparation for their graduation or licensure.
  • Collaborating with other health care professionals to plan, monitor, review, and evaluate the quality and effectiveness of medications and medication regimens.
  • Developing presentations for and presenting to physician practice groups, pharmaceutical manufacturers, payers, and/or employer groups the benefits of OptiMed Health Partners.
  • Perform, interpret, and make recommendations of genetic testing for drug metabolism.
  • Travel medicine consults
  • Provide clinical support for our retail pharmacy site, and assist in the growth and development of new business opportunities
  • Help support and partner with various teams within the organization for clinical growth and opportunity


Supervisory responsibilities


The clinical pharmacist supervises pharmacy technicians and care coordinators, clean room personnel, and pharmacy students and interns when applicable. The clinical pharmacist reports to the Director, Operations and Senior Clinical Officer and/or to the COO.


Qualifications


A clinical pharmacist must have obtained a Doctor of Pharmacy from an accredited institution, passed the NAPLEX exam, and possess a current Michigan pharmacist license including controlled substance license. Pharmacy residency or comparable experience and multiple state licensures is strongly preferred. The clinical pharmacist is a self-motivated team player possessing outstanding written and verbal communication/customer service skills. In addition, this pharmacist possesses advanced clinical knowledge in one or more area(s) of expertise. The pharmacist is responsible for compliance with Continuing Education requirements and maintenance of active pharmacy licenses.


Physical Demands


The clinical pharmacist must be able to spend considerable time viewing a computer monitor. Since the clinical pharmacist may also be required to perform as a staff pharmacist, he/she must be able to spend the majority of the day standing or walking; bending and lifting are required to handle inventory.


Work Environment


The clinical pharmacist environment may vary from an office/desk type confinement to being required to spend most of the day on their feet, working behind the pharmacy counter.
