Pride Health is hiring a Social Worker, Emergency Room to support our client’s facility in Newark, DE. This is a Permanent, Full-Time (Possibility of extension or conversion) opportunity and a great way to start working with a top-tier healthcare organization


Job Summary:

We are seeking an experienced Social Worker to join our Emergency Room Unit for a Permanent, Full-Time assignment. This position plays a critical role in providing care coordination and psycho-social support to patients in a fast-paced, high-acuity emergency care setting.


Key Responsibilities:

  • Work within a Care Management Triad Team model that includes RN Case Managers and Utilization Management staff.
  • Address psycho-social barriers to care and support discharge planning for emergency room patients.
  • Provide emotional support and crisis intervention for patients and families in the ER.
  • Collaborate with multidisciplinary teams to manage patient care, including assisting with the coordination of transfers and admissions.
  • Ensure a smooth discharge process, providing resources and referrals to community services as needed.


Qualifications:

  • Master's Degree in Social Work (MSW) or Master of Social Services (MSS) from an accredited program.
  • LCSW or LMSW certification required prior to start date.
  • Minimum 2 years of acute care experience (emergency room experience preferred but not required).

  • Shifts: 3:00 PM - 11:30 PM Or 11:00 PM - 7:30 AM
  • Salary Range: $62,000.00 to $85,000.00 plus Benefits
