The primary purpose of the Director of Social Services role is to assist in planning, organizing, implementing, evaluating, and directing the overall operation of our facility's Social Services Department. As a Director of Social Services, you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties.



This job will have the following responsibilities:

  • Participate in discharge planning, resident assessments, and developing social care plans.
  • Interview residents/families to obtain social history.
  • Refer residents/families to appropriate community agencies when needed.
  • Provide consultation to staff and community agencies on meeting residents' social/emotional needs.
  • Interpret residents' social, psychological, and emotional needs to the care team.
  • Assist in obtaining resources from community agencies to meet residents' needs.
  • Ensure confidentiality of residents' protected health information.



Administrative Duties:


  • Assist in developing and administering department policies and procedures.
  • Review policies/procedures annually and recommend changes.
  • Complete required documentation, forms, and reports.
  • Assist in arranging transportation for residents when necessary.
  • Provide information on financial assistance programs to residents/families.
  • Assist in reviewing and updating job descriptions annually.
  • Delegate authority and make recommendations to the Director.
  • Standardize methods and provide solutions for residents' social/practical needs.



Qualifications & Requirements:

  • The job requires a Bachelor's degree in Social Work, at least 1 year of supervised experience in healthcare, and Massachusetts Licensure.
