Job responsibilities

  • Evaluation and treating patients.
  • Perform assessments, reassessments, progress notes.
  • Clinic support as needed.


Setting

  • Majority Outpatient


Coverage Hours

  • 40 hours – 5 – 8 hr shifts OR 4 – 10 hour shifts
  • No Weekend Coverage


Site-specific HR credentials required:

  • PT License and license verification
  • CPR training
  • Vaccination record (Current Tdap or declination, documentation of 2 MMR vaccines or positive titer, documentation of 2 varicella vaccines or positive titer, negative QuantiFERON or 2 step ppd in past year), drug screen, background check, HIPAA, CPS COC attestation (attached) ** Out of date vaccinations or test can be completed at site
