Job Summary
We are seeking a skilled and compassionate Physical Therapist to join our team. As a Physical Therapist at Concept Rehab, you will be responsible for evaluating and treating patients with difficulties related to mobility, function, strength, and positioning.
Key Responsibilities
- Evaluate patients for rehabilitation services within 24 to 48 hours of physician's orders.
- Develop effective treatment plans that meet the unique needs of each patient.
- Treat patients according to the treatment plan of care, utilizing evidence-based practices and techniques.
- Supervise the Licensed Physical Therapy Assistant in the delivery of clinical services and interactions with other staff, patients, and family members.
- Communicate with physicians, clinical managers, and other health team members, patients, and family regarding patient progress, problems, and plans.
- Participate in Plan of Care Meetings and Interdisciplinary Team Meetings to ensure comprehensive care.
- Achieve or exceed the minimum productivity standard established by Concept Rehab.
- Participate in in-service training programs for other staff in the facility to promote knowledge sharing and best practices.
- Record treatment notes, progress notes, and provide objective input for discharge summaries, and complete daily billing of services per policy and procedure.
- Instruct patient's families and/or facility staff in maintenance programs to promote patient independence and self-care.
- Participate in discharge planning to ensure a smooth transition for patients.
- Recommend any needed equipment to the appropriate personnel to facilitate improved delivery of clinical service.
- Report any problems with department equipment to the facility personnel to ensure timely resolution.
- Act as a clinical supervisor for affiliating Physical Therapy and Physical Therapy Assistant programs for area universities and colleges to promote education and professional development.
- Consistently portray the mission, vision, core values, cornerstones, and professional image of Concept Rehab, exercising good judgment in the performance of the job.
- Special projects and other duties as assigned.
Requirements
- B.S., Masters, or DPT in Physical Therapy.
- Licensed in the state(s) where services are being provided as a Physical Therapist.
