We are looking for a dedicated Case Manager in Berkeley, CA (and Oakland), to assist within the Human Services industry. We focus on bringing assistance to struggling demographics, and families.
Location: Berkeley, CA Region (multiple sites)
Pay: $22-29/hr DOE (and role)
Shift: Days, Full Time
Description:
-Conduct thorough assessments of clients' needs and develop individualized service plans
-Coordinate and monitor the implementation of service plans, ensuring that goals and objectives are met
-Provide guidance and support to clients, connecting them with appropriate resources and services
-Collaborate with a multidisciplinary team to ensure coordinated and holistic care for clients
-Conduct regular follow-ups with clients to evaluate progress and make necessary adjustments to service plans
-Maintain accurate and up-to-date case records and documentation
-Advocate for clients' rights and entitlements, providing information and assistance as needed
-Stay informed about community resources, social services, and legal requirements relevant to clients' needs
