The Program Manager (PM), PA Navigate, at HealthShare Exchange (HSX) is responsible for managing the full scope of HSX’s activities related to this statewide online closed-loop referral program to connect Pennsylvania residents to needed social services, such as food and housing support, access to healthcare, transportation, employment assistance, and other social determinants of health (SDOH). This position will be classified as full-time remote, but candidates in the greater Philadelphia area who know the local healthcare and/or social services landscape and can be on-site for occasional meetings will be preferred.

Responsibilities

• Program/Business Development: Ensure HSX fulfills its obligations under a grant contract with the PA Department of Human Services (DHS). Ensure adoption of the platform by HSX members (primarily healthcare providers and payers) as well as Community-Based Organizations (CBOs) providing social services. Explore new

markets and guide the program toward financial and operational sustainability.

• Vendor Oversight: Ensure the vendor (findhelp) fulfills their obligations under a contract with HSX and three other PA health information exchanges (HIEs). Monitor vendor delivery against performance criteria and ensure appropriate payment.

• Stakeholder Collaboration: Drive engagement from multiple stakeholders, including DHS, partner HIEs, CBOs, and potential funders or other partners in the community.

• Data Management/Analytics: Ensure that data are flowing appropriately among all interfaced systems. Develop and implement plans to increase use of the system over time. Use social referral and HSX clinical data to demonstrate system/service need, effectiveness, and utility of addressing PA residents’ SDOH needs. Use data to answer strategic questions around where in the state there are gaps in social service capacity, which CBOs are most responsive, and develop insights into how best to meet residents’ social care needs.

• Onboarding/Implementation: Bring new members onboard with the application, and/or supervise the implementation of systems interfaces to ensure data are shared appropriately and within established governance frameworks. Ensure members have sufficient training to use the system effectively.

• Marketing/Communications: Promote PA Navigate through traditional and social media channels. Develop speaking and promotional opportunities at relevant gatherings of stakeholders to promote the use and value of the program. Develop means of reaching the general public who can use PA Navigate directly.

• Risk Management: Identify and manage all risks associated with the program, working with HSX staff and outside stakeholders to minimize threats, maximize the program’s value, and encourage growth.


Additional Responsibilities

• Depending on interests, aptitudes, and availability, this position will also contribute to other strategic programs at HSX, including Data Quality Management , HSX’s Data Aggregator Validation certification from the National Committee for Quality Assurance (NCQA), activities in Population/Public Health, Research, and others as

needed.

• Take periodic evening/weekend on call for mission critical systems.


Qualifications

Required:

• Minimum five (5) years’ professional experience in one or more relevant domains, such as healthcare, social services, care coordination, or population/public health.

• Minimum three (3) years’ experience working with healthcare or social services data in an operations, analytics, data quality, clinical quality, or research capacity.

• Understanding or Experienced in all domains of the Project Management Body of Knowledge (PMBOK).

• Excellent organizational, time management, written and oral communication skills with experience in public presentations, grant/proposal writing, grant/contract management.


Desired:

• Master's degree (or equivalent experience) in health- or science-related field.

• Familiarity with SDOH-related standardized screening instruments, CBO referrals/workflow, and developing data standards (e.g., Gravity Project, NCQA quality measures related to social needs screening/services).

• Knowledge of technology and information systems planning, life-cycle, and implementation.
