Ortho Care Coordinator

A health services organization in New York is currently seeking an experienced licensed Registered Nurse (RN) to join their team in Rockland County as an Ortho Care Coordinator. In this role, the Ortho Care Coordinator (RN) will be responsible for providing optimal quality and continuity of nursing care from the pre-operative setting through a 60-day episode of care.

About the Opportunity:

  • Schedule: Monday to Friday
  • Hours: 8:30am to 5pm
  • Specialty: Case Management
  • Patient Population: Orthopedics

Responsibilities:

The Ortho Care Coordinator (RN) will:

  • Assist the patient throughout the transition of care to ensure their well-being and achievement of the desired outcomes
  • Coordinate and facilitate of services
  • Provide patient and family education
  • Communicate with healthcare providers
  • Perform other duties, as needed

Qualifications:

  • 2+ years of Clinical experience
  • Bachelor's Degree in Nursing
  • NYS Registered Nurse (RN) license
  • Previous experience in Case Management

Desired Skills:

  • Ortho experience
  • PRI certification
