Responsibility: Conducting Social Work Program

Specific Duties:

  • Assess the needs of each participant and develop an individualized plan of care with the multidisciplinary team. The medical social worker will interview and screen all referrals to determine the general the general appropriateness of the prospective participant for the full assessment process and adult day health care participation. The plan of care shall be periodically reviewed as necessary.
  • Provide signed and dated documentation for all service performed the day the service is provided and include signed and dated quarterly progress reports in each participant record.
  • Assumption of responsibility for implementation and coordination of the discharge plan.
  • Provide counseling and referral to outside community resources.
  • Promote peer group relationship through problem-centered discussion group and task oriented committees.
  • Serve as a liaison with the participant’s family and home.
  • Serve as liaison with other community agencies that may be providing services to a participant and work with these agencies to coordinate all services delivered to the participant to meet the participant’s needs and avoid duplication. Liaison shall include, but not be limited to the following agencies:
  • In-Home Supportive Services in the county welfare department
  • Home Health Agency providers
  • Provide discharge planning for all discharged participants
  • Coordinate referrals and services to participants in the evening and weekend Social and Recreational program. The program provides evening and weekend activities and training in social skills to prepare who are developmentally disabled and older and to frail seniors, and seeks to integrate them into existing community based senior programs.
  • Develop individual service plans and evaluations of participants in the Social/Recreational program, and oversee the delivery of activities by recreation staff.
  • Any other job/task assigned by Administrator

Qualifications

  • The Social Worker shall be knowledgeable of the physical, mental and social needs of an adult 18 years and above
  • The professional social worker shall have experience in related areas of social work service delivery.
  • A Master's Degree in Social Work
