Nurse Family Partnership Nurse Home Visitor

The Position: As an NFP Nurse Home Visitor, you will provide essential nursing services to families eligible for the Nurse-Family Partnership program. Upholding the highest standards in nursing practice, you will work closely with clients, medical professionals, and community partners to deliver public health services effectively.

Hours: Full-time position requiring 40 hours per week.

Status: Permanent full-time role with comprehensive benefits.

ESSENTIAL FUNCTIONS
  • Conduct home, office, and remote visits for NFP clients.
  • Maintain fidelity to the NFP model.
  • Manage a caseload of up to 25 clients.
  • Promote public awareness of NFP.
  • Develop community relationships for client referrals.
  • Assist clients in goal-setting and achieving outcomes.
  • Collaborate with professionals to provide comprehensive services.
  • Document nursing care accurately and timely.
  • Participate in quality improvement efforts.
  • Maintain confidentiality and comply with regulations.
  • Support program development and planning initiatives.
  • Encourage a positive work environment and teamwork.
  • Respect differing viewpoints and engage in open communication.
  • Stay informed about healthcare developments for quality service delivery.
  • Respond to emergencies as needed.
  • Complete all assigned duties efficiently.

EDUCATION & EXPERIENCE
  • Bachelor's degree in nursing (BSN) from an accredited institution.


LICENSES, CERTIFICATES & OTHER REQUIREMENTS
  • Active Registered Nurse license in Washington State.
  • Valid driver's license and auto insurance.
