SUMMARY
Family Service League is seeking a full-time Social Worker to provide clinical services to youth and families through our Children and Family Treatment and Support Services (CFTSS) program. CFTSS is a team-based model which is intended to provide families individualized and flexible services by meeting with children in their homes and other community settings. CFTSS provides treatment to children ages 5-21 and enables a greater focus on prevention and early intervention by providing an array of services and the capacity to intervene earlier in children's lives.

RESPONSIBILITIES
- The Social Worker will provide community based mental health treatment to a caseload of 20 children/youth and their families.
- Deliver services with the core values of CFTSS: "Family-Focused, Community- Based, Multi-Systemic, Culturally Competent, and Least- Restrictive."
- The Social Worker will provide evidence-based interventions that are tailored to the child's and family's needs.
- The Social Worker will complete all documentation in accordance with CFTSS standards in an electronic medical record, including notes, assessments, and treatment plans.
- Collaborate with other CFTSS and Care Coordination providers by engaging in regular communication. Prior experience diagnosing using the DSM V and experience with behavioral/therapeutic interventions for children, adolescents, and families required. Proficient computer skills, including Microsoft Office and Electronic Medical Records (EMR) required.
