Community Reintegration Coordinator

Job Responsibilities:

This role involves coordinating services for individuals transitioning back to the community from the Milwaukee County House of Corrections or involved with DOC Community Corrections Probation and Parole. Additionally, it may involve assisting individuals in the general population.



Essential Functions:
  • Conduct orientation sessions for eligible individuals interested in participating in the Prisoner Reintegration Program.
  • Perform LS/CMI (Risk) assessments for eligible individuals.
  • Establish Recovery Support Teams consisting of formal and informal supporters to create a strong support network.
  • Facilitate and organize Recovery Support Team meetings to develop a Single Coordinated Care Plan (SCCP) focused on consumer strengths and needs, ultimately leading to favorable outcomes within a 30-day period.
  • Thoroughly document the SCCP, progress notes, authorization, and crisis plans, collecting outcome data promptly.
  • Monitor service provision and quality through the Recovery Support Team, adjusting the SCCP as needed according to guidelines, while advocating for the consumer.
  • Coordinate or provide transportation as necessary, being adaptable and capable of traveling to various locations for client and agency meetings.
  • Manage a caseload, including regular face-to-face interactions, organizing and attending Recovery Support Team meetings, and engaging in administrative activities associated with the Wiser Choice and Behavioral Health Department.
  • Conduct face-to-face and telephone interviews using required outcome data collection instruments (GPRA) at intake, 6 months post-intake, and program disenrollment.
  • Deliver trainings on the Wraparound philosophy and service approach, supporting consumers in the AODA Wraparound process.
  • Maintain contact with consumers across different community locations.

Other Duties and Responsibilities:
  • Assist in educating and involving consumers in the treatment process.
  • Participate in Recovery Support meetings as necessary.
  • Attend training sessions to enhance facilitation skills.
  • Collaborate with various parties that the consumer interacts with, such as Probation and Parole, DOC, social services, Courts, attorneys, employers, and other relevant entities.
  • Perform any additional duties assigned by the Program Director.
  • Support consumers in accessing AODA treatment and recovery services, which may include mental health, housing, employment, social services, and education assistance.

Work Relationship and Scope:

Directly reports to the Program Director and interacts with a diverse range of individuals, including incarcerated persons, correctional staff, probation and parole staff, support services providers, funders, family members, and the general public.


Job Qualifications:

Minimum Requirements: Bachelor's Degree in Criminal Justice, Social Work, or Human Services. 3 years of relevant experience. Knowledge of Wraparound Philosophy is advantageous. Valid Wisconsin Driver's License, automobile insurance, and access to a car. Proficiency in computer usage and a typing speed of at least 35 WPM. Ability to meet security clearance standards set by the Wisconsin Dept. of Corrections and Milwaukee County.

Proficiency in computer skills is essential.
