The CTI Specialist is responsible for delivering hands-on case management services to clients directly. The CTI Specialist will help clients establish connections to sustainable community support with a focus on five key areas that enhance community stability: psychiatric treatment and medication adherence, financial management, substance abuse control, handling housing crises, and interventions with families. The primary objectives of the CTI Specialist are to maintain client engagement with the CTI program and to ensure the client remains in a stable residential environment during the CTI service duration. This stability and connection are vital in preventing future instances of homelessness.

Summary of the Role

The case manager will assess the necessary resources required to meet each individual's specific needs. They will take all essential steps to ensure the execution of the service treatment plan, including advocating on behalf of the individual to facilitate access to required services; identifying program needs, service gaps, and recommending appropriate strategies. A commitment to RHD values must be evident during the performance of job responsibilities.

Key Responsibilities and Functions

Team-Based Collaboration
  • Collaborate as a member of a diverse team focused on delivering services from a holistic and Values-driven perspective.
  • Assist in forming recovery plans and assessments jointly with the program participant, clinical and medical staff, and other relevant parties as set in conjunction with the program participant.
  • Cultivate and sustain relationships with community organizations to aid program participants in connecting with all available resources.
Direct Care for Clients
  • Provide services in the community to program participants, concentrating on education, facilitating access to resources, social engagement, and achieving goals set in the recovery plan.
Administrative Tasks
  • Complete all evaluations and paperwork as mandated by RHD, funders, and licensing/accreditation bodies.
  • Fulfill any other assigned duties
Requirements
  • Bachelor's Degree in Human Services or a related field.
  • Intermediate level of proficiency in computer skills
  • Preferred experience of at least one year in Case Management
  • Minimum of two years of general work experience providing assistance to individuals in behavioral health, Substance Recovery, or IDD services is essential.
  • Possession of a valid driver's license
Work Environment

Special Working Conditions:
  • Tasks will involve working within the community within the program's service range. Required to transport program participants to appointments.
