POSITION DESCRIPTION

POSTION TITLE: Reentry Case Manager

LOCATION/DEPARTMENT: Pathway Forward Program

REPORTS TO: Program Manager/Case Management

FLSA CATEGORY: Non-Exempt

STATUS TYPE: RFT

OUTCOMES

Assists residents transitioning into the community by providing comprehensive case management services. Works towards reducing the risk of recidivism and promoting a positive lifestyle.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Intake

  • Works with Intake Department to ensure proper setup of resident files and completion of orientation procedures.
  • Undertakes comprehensive assessments and formulation of treatment plans for residents.
  • Collaborates with external agencies for job placement and training opportunities.
  • Ensures compliance with program requirements and guidelines.
  • Monitors residents' progress and provides necessary support.
Employment
  • Assists residents in completing employment requirements and identifying job opportunities.
  • Facilitates referrals to job readiness agencies and training programs.
  • Conducts employment spot checks and ensures residents comply with job placement guidelines.
Financial Issues
  • Assists residents with income reporting, budgeting, and subsistence requirements.
  • Monitors compliance with financial obligations and provides support for delays or issues.
Special Requests
  • Handles special requests and emergencies promptly.
  • Coordinates with agencies for medical insurance coverage and benefits for residents.
Monitoring
  • Ensures residents' accountability and compliance with monitoring requirements.
Case Files
  • Maintains complete and accurate case files for audit purposes.
Home Confinement
  • Facilitates home confinement placement and ensures compliance with program conditions.
  • Assists residents with post-release arrangements and referrals for insurance coverage.
Relocation
  • Processes residents' requests for relocation and collaborates with agencies for approvals.
Discipline Issues
  • Addresses rule violations through disciplinary reports and resolutions.
  • Notifies appropriate staff and agencies for serious incidents.
Release
  • Prepares residents for release by ensuring necessary documentation and follow-up appointments.
  • Completes comprehensive reports on residents' program participation and outcomes.
Staffing
  • Attends regular staff meetings for case management coordination and updates.
  • Adheres to program policies and communicates critical issues to management.
POSITION LIMITATIONS
  • Commits resources as approved and keeps management informed on critical issues.
  • Adheres strictly to program policies and procedures.
PHYSICAL DEMANDS/WORK ENVIRONMENT
  • Requires light physical work in a residential environment.
  • Uses standard office equipment for daily tasks.
  • Maintains a low to moderate noise level work environment.
