Overview:
The Hospice Social Worker's main responsibility is to assist patients and families in addressing psychosocial, emotional, and resource needs during end-of-life situations.
The working hours for this role are Monday to Friday from 8:00 a.m. to 5:00 p.m., including an on-call component.
Recent graduates are welcome to apply.
The salary for this position ranges from $26.00 to $33.50 for LMSWs, along with a sign-on bonus. (Recent graduates and LLMSWs are encouraged to apply, although they might start at a lower rate.) Additionally, they will receive a monthly cell phone stipend and mileage.
Our office is based in Holland, serving Allegan and Ottawa Counties, significant areas of Kent and Muskegon Counties, and the northern part of Van Buren County. The Hospice Social Worker must be willing to drive regularly within this service area.
Key Responsibilities:
- Completing admission and transfer procedures with patients and families, which include explaining hospice services and insurance benefits, as well as handling paperwork accordingly.
- Offering counseling and education to patients and families on the psychological, emotional, and social aspects of death and dying.
- Assisting patients and families in identifying and utilizing suitable resources for environmental, financial, legal, or psychological needs.
- Recording all patient/family interactions and activities accurately, professionally, and promptly.
Requirements for the Hospice Social Worker:
- Must hold a Master's degree in social work from a school accredited by the Council on Social Work Education, with current Michigan Social Work licensure as a clinical social worker.
- Hold a valid Michigan driver's license and have access to transportation.
- Demonstrate sound clinical judgment and the ability to function autonomously.
- Preferably possess knowledge and/or experience in end-of-life care.
- Effective verbal and written communication skills, including concise charting.
