Job Title: Social Worker - Independent Living
Status: Full Time, 32 hours per week
Shift: 8am -4pm

What will I do in this role with Garden Spot?
As a Social Worker, supporting the Garden Spot Communities' mission will include the following responsibilities:
- Provides follow-up to a resident's post hospitalization or transfers from/to another level of care; assessing adjustment to the new living environment, assessing transfer needs or level of care changes with the resident, staff, and family. Arranges support services.
- Serves as Garden Spot Village liaison at community events as needed.
- Serves in a problem-solving capacity, as an advocate for residents. Provides accurate assessments and intervenes with effective treatment plans that empower a resident's coping skills.
- Maintains a written record of the frequency and nature of any qualified social work consultations and services provided or obtained.
- Counsels and provides crisis intervention with residents and/or families as needed.
- Assists residents in resolution of interpersonal and intrapersonal difficulties.
- Assists residents with financial, healthcare provider and insurance questions, making referrals as appropriate.
- Is a resource to other staff on the psychosocial needs and problems of residents and their family members.
- Contacts the hospital social worker and family, where appropriate, to assist in discharge planning for post-hospital care of residents that are hospitalized.
- Provides follow up to emergency response calls and interdisciplinary referrals.
- Provides resident and family support and bereavement services.
- Assists the Marketing Department with pre-entrance level of care/independence assessments as needed.
- Orients new residents to Garden Spot Village services and assists in their adjustment to the routines and schedules. Assists Life Enrichment with Village Life Orientation.
- Assesses problems related to aging, illness, and death and dying, in coordination with chaplaincy services.
- Maintains a positive working relationship with the Social Service Departments of local hospitals, physician's groups and other retirement communities.
What do I need for this role with Garden Spot?
- A bachelor's degree from a school of social work approved accredited by the Council on Social Work Education.
- Experience with geriatrics is preferred.
- Fundamental knowledge of social service programs available to the elderly is a benefit to have and/or willingness to be trained in specific programs utilized in the department.
- Ability and desire to work with the elderly and build relationships.
- Possesses good verbal and written communication skills.
- Proficient computer skills.
- Strong organizational skills.
- Strong conflict resolution skills.
- Compassion and understanding when dealing with challenging circumstances.
- Conveys a professional image when relating to residents and their families.
- Maintains detailed documentation, and preserves the confidentiality of information.
