The Work of the ON CALL Registered Nurse HOSPICE Case Manager Includes
Patient Care
Schedules, assigns and directs staff on appropriate patient visits.
Demonstrates competency in skills required by the agency for patient care.

Communicates significant findings, problems, changes in condition or environment to the CNO, clinical manager, physician, and other members of the care team.

Utilizes the patient's plan of care to maintain continuity of care by communication with appropriate staff.
Maintains flow of communication between in-house staff, field staff and other departments, for coordination of activities and services.
Conducts follow-up with by phone or fax to ensure quality of care and notifies appropriate personnel.
Initiates appropriate nursing procedures.
Works in cooperation with the family/caregiver and care team to meet the emotional needs of said parties.
Develops, prepares, and maintains patient care records with accuracy, timeliness and according to policies. Utilizes a combination of agency resources and rationally recognizes standards of practice to achieve excellent patient care.
Participates in agency performance improvement initiatives.
Utilizes teaching materials in staff and patient/family education.

Gain knowledge of and availability to perform patient intakes and information visits as needed including explanation of benefits/Medicare, complete physical assessment, completion of all pertinent paperwork, and communication of new patient status.

Facilitates development of a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Counsels the patient and family in meeting nursing and related needs.
Provides health care instructions to the patient as appropriate per assessment and plan.

Assists the patient with the activities of daily living and facilitates the patient's efforts toward self-sufficiency and optional comfort care.

Completes, maintains and submits accurate and relevant notes regarding patient's condition and care given. Educates the patient and family/caregiver self-care techniques as appropriate.

Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers.

Facilitates the patient and family/caregiver and other team members in providing continuity of care.
Actively participates in quality assessment performance improvement teams and activities.
Graduate of an accredited school of nursing.

One (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in home care.

Bachelor's degree, with one (1) year of home health care and/or hospice care experience preferred.
Responsible for supervising nursing
nursing skills per competency checklist.
Able to lift supplies and equipment. Visual acuity and hearing to perform required nursing skills.
Ability to provide health care to patients according to patient care plan.
Ability to access, input, and retrieve information from the computer.
Ability to communicate effectively with staff, patients and patient family members, ans management.
Ability to observe, record, assess, and document patient needs.

Through those many years, our nurses have been known for the highest level of skill that continues today in our home health medical and specialty teams, and now with our 2021 SHP Premier Performer Hospice Team.
