Required qualifications:

• Bachelor's Degree in Social Work (BSW) from an accredited school of Social Work.

• Master's Degree in Social Work (MSW) from an accredited school of Social Work, Or Master's level degree in related field with equivalent experience.

• 2 years' experience in an acute care or skilled nursing setting, AND Wide variety of experience in provision of social services/discharge planning, OR Master's / MSW: 1 year experience, preferably in an acute care hospital setting, AND Wide variety of experience in provision of Social Work services.
Preferred qualifications:

• Clinical Social Worker License (LCSW), or license eligible upon hire.

• Experience with Assessing patients, Care Coordination, and Discharge Planning Consult based.

• Knowledge of drug/alcohol abuse treatment options, patients experiencing homelessness, patients with mental health history or in crisis, POLST

• Ability to organize and participate in family meetings.

• Participate in the development of the treatment plan.

• Weekend rotation

• Case Load up to 25 patients
Acute medical Social Work experience performing both roles of Care Coordination and Utilization Review

Enterprise Solutions Inc Job ID # Pay package is based on 10 hour shifts and 40 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
