Job Purpose

The Collaborative Care Case Manager (CCCM) is responsible for carrying a caseload of dually diagnosed intellectual and developmental disabilities /behavioral health consumers who have multiple, complex needs and who would benefit from collaborative case management services to prevent potential crisis situations. The CCCM will provide consultation, training, and technical assistance for individuals with intellectual and developmental disabilities and the staff who support them in accordance with agency policies and operating procedures as well as applicable state and federal statutes and regulations.

Essential Functions
  • Maintains an active caseload of individuals while demonstrating fidelity to the Learning Collaborative Pilot.
  • Ensures the coordination of support meetings and crisis plan development for individuals served through the Learning Collaborative Pilot.
  • Provides home visits and visits to various day and vocational settings as needed.
  • Develops, cross-systems crisis prevention and intervention plans, completes intakes and assessments, intervention and outcome plan for respite admissions or inpatient hospital admissions/discharges and any other documentation of service provided.
  • Provides referrals to skills training and submits documentation of data requirements.
  • Participates in systemic consultation, after-hours crisis support, rotating on-call duty schedule, committees, meetings, and work groups.
  • Ensures response time of one hour telephonically and two hours or less face-to-face.
  • Attends all meetings as requested with leadership, Clinical team, and respite staff, etc.
  • Represents the values and mission of the agency.
  • Maintains linkages and relationships with community partners.
  • Daily participation in triage calls to review any after-hours calls, updates on respite admissions and discussions of programming and referrals.
  • Maintains training and licensure (if applicable).
  • Provides Case Consultation
  • Review records of individuals with IDD residing in a variety of settings including but not limited to their own or family home, group home, or nursing facility to evaluate for gaps in care and advocate on behalf of the individual.
  • Promotes understanding and self-management of behavioral, environmental, and psychiatric challenges for individuals, service coordinators, and caregivers.
  • Identifies and documents interventions to reduce crises for persons at risk of or transitioning from institutionalization.

Minimum Qualifications

Minimum Education: Bachelor's Degree

Defined Education: Behavioral Science or Human Services field

Preferences: None

Substitutions: May substitute a bachelor's degree in any field where there is sufficient coursework (at least 30 semester hours) in human services areas will be accepted 42 Code of Federal Regulations (CFR), § a).

Years' Experience: One (1) year experience

Defined Experience: Personal Intellectual and Developmental Disabilities experience or experience with a family member may be considered.

License/Certifications: Valid Texas Driver's License

Agency Requirements

All staff are required to participate in agency Emergency Preparedness and Environmental Safety programs and may be assigned by their department as a key/essential staff level function during critical events or for the purpose of sustaining business continuity.

This position may require temporary or permanent re-assignment to any MHMR Tarrant facility as determined by program needs and/or the Division/Director.

Assigned work hours may change as the needs of the agency change.

The Functional Title of this position may change as the needs of the agency change.

All work will be completed within the scheduled work hours. All non-exempt (hourly) employees are expected to clock in and clock out for each work shift, no work should be done off the clock.

MHMR reserves the right to change, add to or eliminate positions as it deems appropriate.

Physical on-site presence, including regular attendance and punctuality, is an essential function of this position. Any changes or adjustments to your assigned work schedule or shift hours must be approved by your supervisor in advance.
