Responsibilities:

  • Schedule and coordinate care services to meet client needs and preferences.
  • Communicate effectively with patients, families, and care providers.
  • Maintain accurate and uptodate records of services provided.
  • Assist with onboarding and training of new care providers.
  • Handle patient queries and concerns in a compassionate and professionally.
  • Adhere to company policies, regulations, and guidelines.

Qualifications:

  • Fluent in Spanish and Cantonese.
  • Previous experience in home care coordination or a similar role preferred.
  • Excellent communication and interpersonal skills.
  • Able to provide compassionate care and support to patients.
  • Capacity to remain calm and composed, offering support to clients as needed.
  • Strong organizational skills and ability to multitask.
  • Proficient in Microsoft Office Suite.
  • Must be compassionate and respectful towards patients and their families.
  • Able to work independently and make decisions in line with company policy.
  • Strong attention to detail.
