Our story
Originally founded by two emergency room psychiatrists, Dr. Chris Carson and Dr. Robert Williamson, Connections Health Solutions brings 30 years of experience serving individuals in crisis and operates two of the nation's largest and most studied crisis response centers and is currently expanding to more states. Since opening our doors we have provided care and treatment for hundreds of thousands of individuals in crisis.
ResponsibilitiesWhat You'll Do:
The Lead Case Manager oversees the daily operations of the Case Manager staff in the Observation Unit. Key responsibilities include facilitating admissions, discharge planning, and acquiring necessary collateral. Additionally, this role involves supporting legal processes related to patient care. The Lead Case Manager collaborates closely with the interdisciplinary treatment team to ensure coordinated care and effective discharge planning for patients, their families, clinics, local providers, and other referral sources.
- Manage a caseload on Observation Unit by Facilitating admission triages and implementing an individualized person-centered Treatment Plan that is appropriate for brief intervention. Complete the signed Treatment Plan and review with each patient and any other involved parties. Gather collateral from the patient and involved parties to provide a more thorough assessment of what the patient's immediate needs are for treatment and discharge.
- Monitor available community beds and the observation census.
- Provide daily shift report to Social Services Manager.
- Actively coordinate care for patients to ensure services are coordinated with facility staff. Identify and address gaps in service needs for participating parties in service delivery to the patient and make appropriate recommendations to meet patients' needs upon discharge.
- Collaborate with interdisciplinary treatment team to engage, monitor and communicate with the individual ensuring that safety is maintained while meeting the individual's needs.
- Maintain the clinic record, including documentation of activities performed as part of the service delivery process. Obtain signatures from patients regarding treatment, as necessary. Document all services and patient activities in the medical record regarding brief interventions, coordination of care, discharge planning, treatment planning, etc.
- Maintain a working knowledge of the Involuntary Treatment Act (ITA) and Evaluation and Treatment (E&T) Involuntary Commitment process. Monitor the petition process to ensure timeliness are adhered to, and forms are complete and thorough. When subpoenaed, attend E&T hearings.
- Maintain a working knowledge of King County and MCO system and potential funding sources.
- Participate in individual and group supervision as required. Participate in ongoing education, including in-services, training, and other activities to maintain and improve competency. Demonstrate competency through post-testing, skill observation, and performance as assessed by direct supervisor.
- Performs all other duties as assigned.
What You'll Bring:
- Master's Degree in behavioral health or related field
- REQUIRED LICENSE: Current & unrestricted license as a Licensed Mental Health Professional in State of Washington (LMHC, LMFT, LICSW, LSWA, LCSW, LPC, etc. )
- Knowledge of the Washington State behavioral health system of care and resources within it

The typical hiring range for this role in Washington is $40.50 to $47.50 per hour.
