Description:
Deliver Speech-Language Pathologist services to residents, including assessment, treatment, planning, implementation, documentation, and communication following physicians' orders.

Responsibilities Include:

- Provide Speech-Language Pathologist services to residents and make referrals as needed.
- Conduct regular screenings to assess residents for necessary interventions.
- Evaluate residents to gather data for treatment planning and conduct specialized assessments.
- Adjust treatment plans based on residents' response and physician orders.
- Create customized home or community programs to improve residents' performance.
- Share timely clinical information regarding safety and functional needs for discharge.
- Maintain confidentiality standards.
- Execute treatments as per the plan set.
- Follow management directives.
- Engage in Continuous Quality Improvement initiatives.
- Fulfill assigned duties.

Education and Experience:

- Master's degree in Speech-Language Pathology from an accredited program.
- Completion of National Certification Examination for S.L.P.
- Current state licensure or eligibility for licensure.
- 0-3 years of experience in long-term care.
