Under operational direction of the Manager of Social Work and Center for Grief & Healing this LCSW performs the role of social worker as a member of the palliative care interdisciplinary team (IDT).
The Palliative Care Social Worker completes psychosocial assessments and provides ongoing psychosocial support to patients and families in their homes or in facilities.
This exciting position allows for the team member to work autonomously (days, nights or weekends) ensuring that the routine and admission visits are made to patients within the frequencies identified by the IDT.
