making a difference. A designated intake coordinator assists as the front liner to take a client
inquiry and turn them into an active client. The biggest need here is keeping track of all
prospects until they are no longer a prospect, this is your outreach opportunity, this is your
mission is to be the ultimate resource to community members in need of your dedication. TO BE
the one that is compassionate enough to follow the inquiry case until you can't anymore.
A Field Supervisor Assistant is someone who see's clients and caregiver staff in the field and
checks in to see if the care plan is sufficient for the client, and that caregiver staff is meeting
those needs. Reason we are tossing this out there is you may participate in this side, your
people your connecting resources and help to doesn't need to stop there. You have the
opportunity to stay involved.
You'll be required to work with the Agency Manager, Care Coordinators, Regional Schedulers,
and Recruiter to fulfill the needs of serving the elderly, blind and disabled referral needs in
Western Colorado.

Qualifications:
High School Graduate
At least one (2) year of training and experience in home/health services.
Excellent communication skills – bi-lingual a plus (Spanish)
Must have criminal background check.
Being an organizer, and being task oriented is a have to.

Qualifications that would be a plus:
Seeking someone who has health care experience and/or software and billing experience in the
health care field.
Someone with health care experience is going to be able to diagnose the needs of clients,
assist with training field staff to perform those needs.
Someone with billing experience is going to be able to understand how to set up client payer
sources, and understand how to successfully get the system to bill correctly.

Functional Abilities:
*Must be able to lift and stoop effectively so as to be able to perform the above listed
responsibilities.

  • Must be able to hear adequately with no more than an amplifier on the phone and to speak in
a manner understand by most persons.

  • Must be able to read, write and type 40 wpm or above.

Responsibilities:
Primary Responsibilities (including, but not limited to): Client Intake Assessments

Coverage Assurance:
Assist with Front Desk Schedulers for scheduling of caregivers and
respond to situations where scheduling conflicts, emergencies and/or "call outs" arise.
Answers phones from client prospects, receives al intake information. Including, referral
information, case managers, and payer information.
Travel and sign clients up in their home.
Build care plans or assign a care coordinator to build the care plan. This may require
Huddle meetings with teammates.
Manages all matters related to referrals with the exception of issues that require
professional/service delivery judgement.
Advise appropriate care coordinator and regional schedulers of all new cases to be
serviced.
Documents and reports to care coordinators when there is a change in client status,
change in service requirement, change in staff assigned to case and any unsatisfactory
performance.
Participates in the evaluation of field staff in regard to punctuality, reliability, assignments
and related matters.
Participates in the evaluation of client care plans every 90 days as needed.
Productive and uses time efficiently.
Follows instructions, is punctual and attendance is acceptable.
Cooperates with care coordinators and with client's/family members.
Shows interest, asks questions and seeks information.

Reporting:
Enter necessary quality management and reporting responsibilities and
reports as assigned by the Agency Director.
Performs other duties as assigned, this may include building timesheets, assisting with
product development, or assisting with payer modification/billing adjustment projects, or
simply filing or answering phones and providing customer service.
