About the Role

At UnitedHealthcare, we're committed to simplifying the healthcare experience and creating healthier communities. As a Case Manager - Long Term Services and Supports Specialist, you'll play a critical role in delivering high-quality care to our members.

Key Responsibilities
  • Case Management: Utilize your expertise in Long Term Services and Supports to provide case management services to our members, ensuring they receive the necessary support and resources to maintain their health and well-being.
  • Intake and Assessment: Conduct thorough intake and assessment processes to identify the needs of our members and develop personalized support plans.
  • Service Coordination: Coordinate with healthcare providers, community resources, and other stakeholders to ensure seamless delivery of services and supports.
  • Monitoring and Evaluation: Regularly monitor and evaluate the effectiveness of support plans and make adjustments as needed to ensure optimal outcomes.
  • Communication and Collaboration: Maintain open and effective communication with members, families, and healthcare providers to ensure a collaborative approach to care.
Requirements
  • Education and Experience: Meet one of the following requirements: Bachelor's degree in a human behavioral science field, 5+ years of relevant experience in LTSS, or a combination of education and experience.
  • Technical Skills: Proficiency in MS Office, including Word, Excel, and Outlook.
  • Travel and Transportation: Access to reliable transportation and the ability to travel up to 75% within assigned territory to meet with members and providers.
Preferred Qualifications
  • Experience in LTSS: Experience as a caseworker or case manager with an LTSS population, in a private or public social services agency may substitute for the required education on a year-for-year basis.
