Responsibilities
  • The pharmacist is a part of the healthcare delivery team and is responsible for the use of medications throughout the hospital.
  • Modifies medication dosages as determined by age and weight, renal function
  • Correctly interprets/documents orders and intervenes on problem medication orders/drug interactions, ADRs, DUEs, errors
  • Maintains accountability with drug use, including controlled drug inventory and management of hazardous waste
  • Performs aseptic preparation of sterile products
  • Participates in clinical pharmacy initiatives, patient teaching, code support, effective use of dosing protocols, tracking of clinical interventions
  • Supervises the care provided by licensed and unlicensed personnel, in accordance with the current Idaho Pharmacy Practice Act
  • Provides coverage in the central pharmacy and clinical pharmacist shifts, as needed
Qualifications

Job Requirements:

  • Graduate of an accredited School of Pharmacy
  • Maintains current pharmacist license in Idaho
  • Maintains current BLS, ACLS, and PALS certifications
  • Possess computer skills for order entry, word processing, clinical interaction
  • Experience with drug use evaluations/monitoring, clinical interventions
  • Collaborate with other clinicians and healthcare providers to implement medications for effective disease state management
  • Experience in sterile products preparation, including TPN and Chemotherapy

Preferred Job Requirements:

  • Five to seven years of hospital pharmacist experience
