PURPOSE:
The purpose of this job is to fill open shift needs. This position involves working at different locations with different clients as assigned.

Employee will be expected to work up to 45 hours per week between 2PM and 10PM, Wednesdays through Sundays .

Employee may accept additional work outside of these hours for the current DSP hourly rate depending on the shift, with the prior approval of the Supervisor and Director.

ESSENTIAL DUTIES AND RESPONSIBILITIES included, but not limited to:

Provide training and support in all daily living skills including but not limited to nutrition, personal hygiene, health care, safety, communication, interpersonal relations, mobility, financial management, home management and use of leisure time as outlined in the individual service plan (ISP).Ensure individuals are taking medication according to prescribed schedule.

Accompany individual and facilitate participation in visits for medical care, therapies, personal shopping, recreation and other community activities as needed.

Provide training or assistance in meal preparation, shopping, laundry, housekeeping, simple household repairs, and financial and medication management as needed.

Provide training and support in the areas of social, emotional, physical and spiritual development.

Become acquainted with individual's job setting and key personnel supervising the individual (including job coach, support coordinator, day program, etc.).Provide transportation in privately owned car or by agency owned vehicle for individuals as directed by Administrative Coordinator and/or Residential Manager.

Maintain and submit all required paperwork in a timely manner. (i.

e:
time sheets, expense reports, daily documentation paperwork).Attend and participate in Mandatory DD Team Meetings and trainings, including cross-training, and other events as required.
Complete ongoing mandatory online training modules and maintain all required certifications as per job regulations.

Communicate regularly with supervisor, Residential Manager (if applicable) and Residential Services Manager (as needed), on individual's progress, issues and concerns as they arise.

May be assigned to perform administrative or other agency-related duties at the discretion of the Administrative Coordinator and/or Director depending on the scheduling needs of the individuals in the program.

Other duties may be assigned. RequirementsTo perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. The ability to organize, mediate, to be creative, multitask, empathetic, and willing to learn. This includes the development of plans, completion of assessments and daily documentation.

Must have knowledge of counseling techniques, behavior modification strategies, medical and psychological aspects of disabilities, able to use the English language, and communicate clearly.

Must be able to manage time effectively, to work alone and as a team member.

Must have a Georgia State Driver's License and insured privately owned vehicle and be willing to travel to work anywhere within the agency's regions of service.

Ability to work with any supported individual and their team.

Must carry an agency cell phone and respond to calls from Staffing Coordinator and/or Program Manager beginning one hour before and lasting through the end of their shift, following agency policy on cell phone usage.

May be required to attend meetings and professional trainings outside of assigned schedule with at least two weeks' notice. Online training will be completed within regular shift.
