Hospice and Home Health Registered Nurse

Sunbelt is currently seeking a Hospice and Home Health Registered Nurse for a contract position with a local client in Georgetown, KY. If you have at least one year of recent home health experience, we encourage you to apply for this opportunity.
Responsibilities:
- Conduct admissions, discharges, resumption of care, and transfers using OASIS
- Manage a caseload of 30 units per week within a 40-hour work week
- Coordinate care with other disciplines, Physicians, Caregivers, and Insurance Companies
- Implement, develop, document and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
Experience:
- 1-2 years experience as a Registered Nurse in a clinical care setting - Required
- OASIS Experience - Required
- Homecare Homebase Software Experience - Required
- Experience in case management, handling at least 5-7 patients per day and 25-30 per week
Education:
- Associate's Degree of Nursing or Bachelors Degree of Nursing
- Kentucky Registered Nurse License or compact license
- American Heart Association Basic Life Support certification
Work Hours: 8:00 AM - 5:00 PM, 40 hours per week, with some on-call and weekend requirements
