The Medical Social Worker assists the patient and family with emotional, social, and financial needs that are identified in the assessment.

Serves as a liaison in the coordination of community resources. Participates in the bereavement and educational program and provide consultation to the staff, volunteers and/or community as needed.

Qualifications


Medical Social Worker degree in Social Work, Masters Degree or Bachelors Degree in social work or sociology from an accredited school of social work required.

Current social workers license or certificate as required by the State of Oklahoma.
At least one (1) year experience in the community or in-home care preferred.
Excellent verbal and written communication skills.
Must maintain a working cell phone.
Knowledgeable of dying process.

Must have a valid drivers license, reliable vehicle, maintain valid auto liability insurance, and be able to drive throughout service area under a variety of weather conditions.

Responsibilities

Completes intake/referral form and coordinates with Clinical Coordinator or Administrator to ensure timely admissions.
Completes initial admission process as needed.
Performs psychosocial evaluation of patient, family, or significant other which reflects planned interventions and goals for problems identified. Completes assessment within seven (7) working days of admission.

Assesses and develops social worker Plan of Care, identifying problem areas as defined by the patient, the unit of care, the Interdisciplinary Team, and the social worker and updates as necessary.

Counsels patient/family on psychosocial issues of death, dying, and bereavement. Promotes supportive family involvement when possible.
Works with patient/family in mobilizing personnel and community resources. Serves as an advocate, makes appropriate referrals to agencies and demonstrates proper follow-up.
Evaluates patients insurance coverage and options when appropriate.
Creates, maintains, and utilizes a current community resource file.
Acts as a liaison between patients and their caregiver and needed community resources so that appropriate services/entitlements are accessible.
Demonstrates an understanding and sensitivity to the needs of the geriatric and hospice population and relates effectively to them.
Attends weekly Interdisciplinary Team meetings, staff meetings, and participates in other such appropriate meetings and committees.
Assists team members with psychosocial aspects of care to ensure direction and continuity.
Maintains current, accurate, legible documentation in patient records and turns in paperwork within 24-hour (next working day) time frame.

Accepts assignments for on-call as needed for crisis intervention.
Assists in discharge planning from hospice service when care is no longer appropriate.
Attends in-services and meets minimum requirements (12 hours) for yearly in-service hours.
Takes responsibility to keep personnel file current with required information.
Participates in agency and community programs as requested to promote professional growth and understanding of hospice care.
Demonstrates familiarity with policies and procedures of Accentra Home Health and Hospice and State and Federal regulations.

Reports to work on time and communicates with Administrator or designee when late or not able to come to work.

Complies with dress code and projects a professional attitude toward self, patients, and other staff members.

Follows Accentra Home Health and Hospice public relation policies regarding outside speaking to professional affiliations, clearing all such requests through the Administrator.

Accepts other assignments as appropriate.
