Make a Difference
Thediabetes educator provides an educational experience for patients and their significant others to promote effective diabetes self-management.

The role of the diabetes educator is multidimensional with boundaries for accountability to self, patient, profession and other members of the health care team and some responsibilities include assesses patient strengths/educational needs using available data and resources; collaborates with team to establish/prioritize individualized plan with patient/significant other; incorporates generally accepted technology and knowledge into patient education experience based on standards of practice for own discipline, ADA, AADE and other regulatory agencies; promotes safe practice of diabetes self-management related to medication administration, meal planning, exercise, blood glucose monitoring and other aspects of care; empowers patient/significant other with knowledge and skills in self-care for optimal health and psychosocial adaptation to maintain/improve quality of life; consults with other health care professionals and organizations to utilize appropriate resources (human, financial and material) which promote consistency and continuity of care of high value; and other related tasks as needed.

The Diabetes Management clinic is located on the north side of Indianapolis, Indiana, however some travel will be required to the East side of Indianapolis and Carmel.

Exceptional Skills and Qualifications

  • Graduate of National League for Nursing (NLN), Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), or National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA) accredited school of nursing, or three years of related professional nursing experience.
  • Licensed as a Registered Nurse (RN) with a valid license to practice in the state of Indiana as listed in the Nurse Licensure Compact (NLC).
  • Two years or more of nursing experience required.
  • BSN required.
  • Certified as a diabetes educator preferred before hiring.
  • One to two years of educating patients in the area of diabetes preferred.
  • Excellent team skills.
  • Compassionate and caring.
  • Accountable and self directed.
