Job Summary:
As a Permanency Coordinator, your role is to help establish lifelong connections for youths to support their successful transition back to their home communities.
You will need to develop a deep understanding of family histories, assess available resources, and provide support to both families and communities during the treatment process.
Your responsibilities include representing students in court, coordinating with families and referral systems, attending team meetings, and maintaining positive relationships with stakeholders.
Documentation, progress reports, and discharge summaries are also part of your duties as you work to ensure a smooth transition for each student.
Duties and Responsibilities:
- Provide thoughtful clinical interventions
- Coordinate family contact and medical appointments
- Engage in psychoeducation and support students' needs
- Develop intervention plans and identify safety risks
- Document services and ensure timely reporting
- Maintain professionalism and seek continuous improvement
- Establish rapport with students and uphold boundaries
- Support students in program activities and decision-making
Qualifications/Requirements:
- Minimum 2 years of experience with intense individuals
- Bachelor's degree in relevant field
- Valid driver's license and 21+ years of age
- Successful completion of background checks
