Job Description

PRIMARY FUNCTION:

To provide direct Care Management Services, to a defined patient population, while supervising the Case Management & Social Work staff. The Care Management Supervisor offers continuous support to the department, collaborating closely with Managers, Directors, and the Vice President to enhance the Care Management process. Responsibilities include overseeing department teams, coordinating projects to enhance patient and staff satisfaction, reduce length of stay, and improve service delivery.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

  • Maintains assignments; resolves team issues within the Care Management team.
  • Formulates discharge plans with patients, families, and healthcare team members for post-acute care.
  • Assesses patient's needs comprehensively and develops appropriate care management plans.
  • Supervises department staff, providing orientation, mentoring, coaching, and evaluations.
  • Coordinates training programs and ongoing educational sessions for staff.
  • Manages employee files and interacts with various internal and external stakeholders.
  • Assists in budget preparation, policy development, and departmental performance improvement.
  • Prioritizes requests and forwards them to higher management as necessary.
  • Performs other related duties as assigned.

EDUCATION AND EXPERIENCE REQUIREMENTS:

  • Master in Social Work (or related field) or BSN in Nursing (Masters preferred).
  • 5-7 years minimum experience in healthcare, with at least 2 years in a leadership position.
  • Equivalent combination of education and experience may be considered.

SPECIAL REQUIREMENTS:

  • Licensed Social Work credential preferred before June 11, 2024 (required in Delaware by this date).
  • CCM/ACM Certification preferred, completion within one year of hire if not certified.
  • RN license required in Delaware.
