  • The purpose of this position is to assess residents' physical and mental function and document data on minimum data set forms completely and accurately; document all additional assessments required completely and accurately; and determine appropriate referrals to other health care professionals; and to use the resident assessment protocols to determine whether to proceed or not proceed. This position will be responsible for the overall clinical oversight of the residents in his/her assigned unit. To Identify a key person on the infection prevention team having responsibility for the infections prevention program, dissemination of infection prevention information including surveillance data and policy decisions. Job Overview
  • Interview residents, residents' representatives and others with information related to the minimum data set assessment.
  • Collect all data necessary to complete minimum data set forms and related documentation.
  • Make appropriate referrals to other health care professionals.
  • Participate and/or conduct interdisciplinary meetings.
  • Supervise and participate in the overall care of the residents
  • Oversee clinical coordination, review labs, assist the physician with rounds
  • Participate in the facilities Quality Assurance process to review facility systems as directed
  • Effectively direct, manager, and operationalize the infection prevention program Requirements for this position
