Summary:


VA Illiana Health Care System is looking for full-time Family Practice or Internal Medicine trained physicians to join the Danville, IL clinic.

The ideal candidate will provide comprehensive care under the Patient Aligned Care Team (PACT) model, focusing on patient education, preventive care, and collaboration within an interdisciplinary team.

Responsibilities include managing chronic conditions, evaluating and treating medical issues, disease prevention activities, and providing clinical oversight to primary care mid-level providers.

Physicians are expected to participate in primary care staff meetings, attend to timely documentation, and meet professional care standards.



Qualifications:
  • U.S. citizenship
  • Doctor of Medicine degree or equivalent
  • Full license to practice medicine or surgery
  • Completion of residency training program approved by the Secretary of Veterans Affairs
  • Proficiency in English

Preferred Experience:

Board Certification in Family Medicine or Internal Medicine and 1-2 years in Primary Care.


Duties:


  • Provide comprehensive medical care under the PACT model
  • Manage chronic conditions and inter-current medical issues
  • Collaborate with PACT for patient care
  • Conduct disease prevention activities and mental health screenings
  • Supervise primary care mid-level providers

Physicians must ensure timely documentation and address clinical reminders appropriately to maintain professional care standards.
