Exciting Opportunity to Join Our Team as a Personal Care Specialist at Element Care

OVERVIEW:

As a Personal Care Specialist at Element Care, you will be an essential member of the PACE Interdisciplinary Team, dedicated to ensuring the well-being and safety of our participants. You will work under the guidance of a licensed nurse, actively engaging in the planning, execution, and assessment of personal care services for individuals at our center and within the community.

KEY RESPONSIBILITIES:

  • Provide comprehensive personal care support to participants both on-site and off-site.
  • Assist with meal services, adhering to dietary requirements and offering encouragement for dietary compliance.
  • Aid in specimen collections, vital sign monitoring, and other nursing activities as needed.
  • Communicate any changes in participants' medical, cognitive, or functional status to the primary nurse promptly.
  • Accompany participants to medical appointments and other essential outings.
  • Execute daily assignments efficiently and maintain cleanliness and organization of facilities.
  • Participate in activities, projects, and one-on-one therapy sessions.
  • Ensure the safety of participants during vital sign checks and transfers.
  • Stay up-to-date on training requirements and attend mandatory meetings.
  • Follow all established policies and procedures, including CPR training and fit test evaluations.
  • Perform additional duties as assigned.

REQUIREMENTS:

  • Preferably hold a High School diploma or equivalent.
  • Possess HHA certification or equivalent qualification.
  • Minimum of one year of experience working with the geriatric community.
  • Ability to interact compassionately with older individuals and cater to their needs effectively.
  • Skilled in vital sign assessments, participant transfers, and communication regarding participant well-being.
  • Demonstrate strong verbal and written communication skills.
  • Collaborate effectively with team members in a positive manner.
  • Basic proficiency in Microsoft applications is advantageous.
  • Frequent local travel may be required for this role.
  • Must have received the Covid vaccine.
