  • What You'll Do:
    • Deliver direct patient care as per State Nurse Practice Act
    • Follow current nursing practices based on assessments and care plans
    • Ensure accurate and timely documentation of patient services
    • Offer patient and family education tailored to their needs
    • Engage in coordinating hospice services and reporting requirements to the interdisciplinary team
    • Manage pain and symptoms effectively, assessing patient responses to treatments
  • Qualifications:
    • Graduate from a recognized practical nurse or vocational nursing program with a valid state license
    • Prior experience in home health or hospice care preferred with minimum medical/surgical experience of 1 year
    • Active CPR certification
    • Valid driver's license and insured vehicle in working condition
