Utilizing a collaborative approach, the Case Manager will assess, plan, implement, monitor and evaluate the options and services required to meet an individual's health needs.
Provides comprehensive on-going case management services to patients by coordinating and managing the care of patients to meet multiple service needs across the continuum of care.
Ensure optimal patient outcomes that address quality, service, customer satisfaction and cost-effectiveness.The Case Manager will partner with the multidisciplinary team to establish care and allocate resources associated with the patient's risk assessment.
Assists the patient/patient's family in coping with illness by optimizing the patient's/family's self-care abilities and supporting their consumer rights.Medical University Hospital Authority (MUHA)

Job Summary/Purpose : The Nurse Case Manager (NCM) I reports to the Manager of Case Management and Care Transitions.
This role coordinates a plan of care to achieve clinical outcomes and promotes a safe and timely discharge.This position manages the plan of care throughout the care continuum to ensure the efficient movement of the patient through the hospital system.
The Nurse Case Manager I serves as a clinical expert related to discharge planning, resource utilization, and case management.Minimum Training and Education:
Bachelor of Nursing degree preferred with minimum of one year of experience.
RN staff hired on or after July 1, 2013 with an Associate or Diploma degree in nursing are required to be enrolled in an accredited BSN program within two years and successfully obtain a BSN degree within four years of the RN hire or reclassification date.
Refer to policy A141 for more details. One year of case management and/or utilization review work experience preferred.Required Licensure, Certifications, Registrations:
Case Management certification preferred.
Current Basic Life Support (BLS) or ACLS required, either a certification from an American Heart Association (AHA) BLS for Healthcare Providers (or AHA recognized equivalent) or an American Red Cross CPR/AED for Professional Rescuer and Healthcare Provider.
