Nurse Case Manager (NCM) I

Company: Medical University Hospital Authority (MUHA)

  • Reports to the Manager of Case Management and Care Transitions
  • Coordinates a plan of care to achieve clinical outcomes and promotes a safe and timely discharge
  • Manages the plan of care throughout the care continuum
  • Serves as a clinical expert related to discharge planning, resource utilization, and case management

Minimum Training and Education:

  • Bachelor of Nursing degree preferred with minimum of one year of experience
  • One year of case management and/or utilization review work experience preferred

Required Licensure, Certifications, Registrations:

  • Case Management certification preferred
  • Current Basic Life Support (BLS) or ACLS required

Scheduled Work Hours/Shift:

  • Days, 8:00am to 4:30pm
  • Occasional weekends & holidays, no nights
  • $5k sign-on bonus for external candidates with 2-year commitment

Fair Labor Standards Act Status: Job Summary/Purpose: The Nurse Case Manager I role involves coordinating care plans, ensuring patient flow through the system, and offering expertise in discharge planning and resource utilization.

The role requires a Bachelor of Nursing degree and previous case management experience is advantageous. A Case Management certification is also preferred.
