POSITION SUMMARY


The Assistant Health Services Director assists the Health Services Director in ensuring resident care needs are met by health andsupportive services provided to residents as directed by health services leadership.

Assists healthservices leadership in overseeing health services procedures and systems, care staff, andschedules.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Partners with health services leadership to create a resident-centered approach to the planning anddelivery of services to residents, supporting residents' rights to participate in decision-making andtreating residents with dignity and respect.
  • Assists with assessments or evaluations on health, functional and psychosocial status of residents.
Participates in the development of individualized service plans. Proactively manages care andservices for each resident, maximizing the resident's opportunity to remain in the assisted livingenvironment. Problem-solves as part of the management team to case manage and meet theneeds of residents.

  • Evaluates residents with reported changes of condition. Provides team member direction, serviceplanning, and coordination of care to meet the residents changing needs.
  • Collaborates with the management team on the move in of prospective residents, taking intoconsideration the needs of the prospective resident, current community acuity, and staffing levels.
  • Assists in the coordination of resident health services appointments for offsite visits throughtransportation services or use of the company vehicle.
  • Communicates regularly with residents' families regarding health care needs. Provides education andsupport to residents and families.
  • Assists in supervision and training of health services team members including personal care andmedication management.
  • Partners in the investigation and root cause analysis of incidents. Collaborates with the managementteam on the development and implementation of appropriate interventions.
  • Collaborates with Health Services Director to ensure the documentation and communication ofresident care and services. Ensures resident records are completed in a timely manner and maintainedaccording to company policies and state regulations.
  • Assists in the management of Infection Control. Provides training to staff on infection controlpractices, investigates, and reports incidents of infection and tracks infection data.
  • Provides on-call support and phone triage for team members' questions regarding resident concerns.
Work as a Medication Aide and/or Personal Care Attendant as needed.

  • Performs other duties as assigned.

QUALIFICATIONS AND EXPERIENCE

Must have a current, unencumbered LPN/LVN license. Excellent written and oral communication,computer and motivational skills are essential to success. Ability to creatively problem-solve in bothresident care and employee management situations. Experience in geriatric nursing, medicationadministration, case management, training/staff development, budgets, and team management aplus. Additional course work in management an asset.

EDUCATION

Associates or Bachelor's degree in nursing or equivalent knowledge and skills obtained through acombination of education, training, and experience
