Synchrony Rehab, a subsidiary of Trilogy Health Services LLC, is seeking a licensed PRN Occupational Therapy Assistant at The Springs of Mooresville, a dynamic and innovative Skilled Nursing and Assisted Living health campus located in Mooresville, IN.

You must hold an active or pending Occupational Therapy Assistant (COTA) state license to apply for this position.

Responsible for providing, under the direction of an occupational therapist, therapeutic interventions in an interdisciplinary environment consistent with the position's qualifications, professional practices and ethical standards.

Demonstrates accountability for and contributes to program development, quality improvement, problem solving and productivity enhancement in a flexible interdisciplinary environment.


ROLE AND RESPONSIBILITIES

  • Provides age-appropriate therapeutic interventions as directed by the occupational therapist in campus locations, home health and/or outpatient care.
  • Evaluates patient response to treatment and provide feedback to the rest of the team.
  • Documents the course of patient care including progress made and continuing need for treatment.

  • Participates in survey preparation activities.
  • Collaborates with the occupational therapist to develop new methods of delivering education to patients and family.
  • Assists the occupational therapist in providing team members with in-service/cross-training on occupational therapy knowledge and skill areas as needed.
  • Contributes to the delivery of cost-effective care.
  • Travel, as needed, to other campus locations and/or to home health assignments in the designated territory.
  • Reports any complaints and/or grievances made by the residents to the Rehab Program Director or Director of Health Services. Reports incidents or suspected incidents of resident abuse to the Rehab Program Director/Director of Health Services and/or the Executive Director immediately.
  • Complies with all resident rights per the company's Resident Rights Policy.
  • Maintains professional/technical competencies and proficiencies for job responsibilities.

QUALIFICATIONS AND EDUCATION REQUIREMENTS

  • AOTA approved associate degree program or equivalent training that meets all state practice requirements and hold an applicable, unrestricted license to practice.
  • Two (2) years' experience in a healthcare, senior living industry or long-term care environment preferred.
  • Exemplary computer skills that include knowledge of the Microsoft Office Suite of products.
  • Must hold an applicable license and unencumbered license.
  • Must maintain current CPR certification for healthcare providers through CPR training that includes hands-on practice and in-person skills assessment.

PHYSICAL REQUIREMENTS
Sitting, standing, bending, reaching, stretching, stooping, walking and moving intermittently during working hours. Must be able to lift at least 50 lbs. and assist and stabilize residents as they transfer from equipment.

Must be able to see and hear or use prosthetics that will enable these senses to function adequately to ensure that the requirements of this position can be fully met.

Must be able to properly operate office equipment.

Must be able to maintain verbal and written communication with co-workers, leadership team, supervisors, residents, family members, visitors, vendors and all business associates within or outside the Health Campus.
