JOB SUMMARY

Assesses, plans and implements the nursing care of residents within the patient care unit settings with the supervision of a Registered Nurse.

Responsible for ensuring continuity of care of the residents between shifts by providing direct care as well as supervising the care given by NAC's and supportive staff members.

Participates in resident and family teaching as determined in scope of license. Maintains role as patient advocate with a focus on the district's mission.

ESSENTIAL DUTIES AND RESPONSIBILITES

  • Gives a thorough report to oncoming shift by participating in end of shift report process.
  • Follows through as needed on information given by shift report, resident or family regarding resident concerns.
  • Implements plan of care of the resident based on assessments and goals established by the interdisciplinary care team.
  • Treats residents and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of residents and their families.
  • Supervises other members of the healthcare team as needed to accomplish the plan of care which has been developed.
  • Demonstrates competence in technical and manual skills according to scope of practice for LPNs/LVNs.
  • Functions as a resident advocate by protecting the resident's rights.
  • Demonstrates creativity in solving problems and looking at situations in new and innovative ways.
  • Interacts with residents, families, co-workers, managers and other departmental staff in a kind, knowledgeable and professional manner.
  • Consistently involves resident and family in planning for care, treatment, teaching needs and discharge plans.
  • Participates as needed in resident care conferences and updates plan of care, as appropriate.
  • Functions as a leader for the NAC's; delegates in a manner that contributes to the excellence of the nursing unit to which LPN/LVN is assigned.
  • Participates in orientation of new staff after preceptor training. Demonstrates ability to remain calm in emergency and/or stressful situations.

JOB KNOWLEDGE & QUALIFICATIONS
Education and/or Work Experience Requirements

  • Graduate of an accredited LPN/LVN program.
  • Current Licensed Practical Nurse/Licensed Vocational Nurse in State of Washington.
  • Current AHA BLS certification.
  • ctive ACLS and PALS certification, or agreeable to take the district led classes within six months of hire.
  • Current certification in IV therapy, or agreeable to participate in the district IV competency training program with the Nurse Manager within 6 months of hire.
OTHER SKILLS & ABILITIES
Language Skills
Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports and business correspondence. Ability to effectively present information and respond to questions from groups of managers, patients, customers, and the general public.
Mathematical Skills
Ability to work with mathematical concepts such as probability and statistical inference. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical solutions.
Reasoning Ability

Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.

Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
