We are seeking a skilled Rehabilitation Nurse Specialist to join our team at Cynet Health and Travel Nurse Source. As a Rehabilitation Nurse Specialist, you will play a critical role in assisting individuals with disabilities and chronic illnesses to attain and maintain maximum function.

Key Responsibilities:

  • Assist clients in adapting to an altered lifestyle while providing a therapeutic environment for client\u2019s and their family\u2019s development.
  • Complete referrals and take physicians\u2019 verbal orders as necessary in a timely manner.
  • Perform physical assessments, evaluate patients\u2019 needs, and consult with physicians to develop a plan of care and documentation.
  • Assess the need for additional health or ancillary services and refer patients for service(s) when appropriate.
  • Assess family and home environment and make recommendations to enhance patient care and promote safety.
  • Involve patients and/or family in developing and revising an individualized care plan, including necessary teachings with realistic goals.
  • Coordinate the multidisciplinary care team and plan for patient care and document appropriately.
  • Interpret admission, discharge, and other policies of the company, physician, family, other healthcare providers, and community as needed.
  • Provide clear, written, and verbal Home Health Aide and LPN assignments updates.
  • Provide skilled nursing care and assessments to patients in the rehab environment, according to the plan of care and promptly revise the plan as necessary.

Requirements:

  • Graduate of an accredited nursing program.
  • Current nursing license, in good standing.
  • Minimum of one year current rehabilitation/skilled nursing experience.
  • Current AHA BLS, ACLS if unit consists of Telemetry.
