Brookside Community Health Center was founded in 1970 through a Model Cities grant, with the aim of providing community-centered, affordable healthcare for the surrounding neighborhoods.

Today Brookside continues this mission as part of Brigham and Women's Hospital, serving a predominantly Spanish-speaking, MassHealth-insured patient population, utilizing a robust team-based approach to provide wrap-around care for our patients who need it most.

Brookside Community Health Center is a comprehensive health center with primary care and specialty services on site, including dental, nutrition/WIC, lab, and behavioral health.

We are licensed by Brigham and Women's Hospital (BWH), a division of Mass General Brigham (MGB) Healthcare system.

Our mission is focused on compassionate, high-quality, team-based care for all patients and families, with a particular focus on vulnerable populations.

A large percentage of our patients are insured by MassHealth (Medicaid) and live within the surrounding communities of Jamaica Plain, Hyde Park, Mattapan, Dorchester, Roxbury, Roslindale, and West Roxbury.


Our patient population is largely comprised of first- and second-generation immigrants, predominantly Spanish-speaking, hailing from the Dominican Republic, Puerto Rico, and Central America.

Our commitment is to ensure language-concordant, fair access to quality healthcare for all.

We have a team-based care model as a Patient-Centered Medical Home, with integrated behavioral health, team RNs, population health coordinators, high-risk RN case management, community resource specialists, community health workers, as well as nurse-led visits and non-face-to-face visits with support of My Chart and/or RN phone calls.

Brookside includes a strong interdisciplinary team providing care for patients with substance use disorder, including Medication-Assisted Treatment. We value wellness across the health center, for all our staff, providers, and our patients. We continue to work on initiatives to improve wellness. This role offers more than traditional Primary Care. You'll delve into patient-focused practice while embracing a team and value-based population health management approach.

Our team of dedicated Primary Care Physicians emphasizes proactive health screening, comprehensive evaluation and diagnosis for symptoms, and expert management and treatment of conditions.

For our patients with complex conditions, we provide care coordination and enhanced communication with multiple specialists who are part of our patients' care teams.

Qualifications:


  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited medical school.

  • Current, unrestricted license to practice medicine in Massachusetts including MCSR and DEA.

Appointment at the Harvard Medical School

Depending upon the position, there may be opportunities to teach residents and medical students. All Harvard Medical School Faculty appointments require at least 50 hours of teaching per academic year. Harvard Medical School faculty appointment is not a requirement for this position. Appointment at the Harvard Medical School will be commensurate with experience, training, and achievements in addition to teaching activities.
